can someone please help me with this??? I have to prioritize these nursing DX i know how but some of them are alike and I am stuck. I have to list them in priority of importance.. these are on a well newborn....
they are : risk for ineffective airway clearance r/t increased mucus
risk fro ineffective breathing pattern r/t fatique
risk for aspiration r/t immature gag reflex
risk for impaired gas exchange r/t collapsed aveoli
risk for fluid volume deficit r/t poor suck
risk for altered nutrition
risk for altered thermoregulation r/t large body surface area
risk for injury r/t misidentification for helpless infant
impaired skin integrity r/t interuption of nerves tissues and blood vessels
knowledge deficit cord care r/t mothers inexperience
acute pain r/t interuption of nerves and blood vessels and tissues
please help!!!!!

We were taught to priortize first by ABC's (airway - breathing - circulation), and then by Maslow's hierarchy of needs. Good Luck! :)

Haven't had any OB/GYN, but the others are right. Like my instructor said, "If they aren't breathing, nothing else matters." I lack the knowledge to prioritize those but definitely the first 4 would be at the top of the list.

Always go with the ABCs ! Others are secondary -- I think Maslow's hierarchy is a good guide. I love the "if they aren't breathing nothing else matters!!" Am going to use that to remind my orientees! Hope I don't have to pay someone royalties for its use!:)

Oxygen, Pain, Foods/Fluids, Asepsis(Safety), etc. Maslow's Hierc. is the place to guide you in your prioritization. Oxygen needs are always first and in writing a care plan for class, use the most pertinent diagnosis (the one needing the most attention and intervention) to write your interventions on. You probably don't need to do all of those airway diagnoses. Also always do actual problems instead of at risk for problems. I hope this helps.

thank you so much for your help , I have my care plans done we just had to make a list of dx and prioritize them and turn them in with the care plans .. it had to be a list of 10 - 12 to turn it with the care plans.. thanks so much....
