Hey Everybody
I am having the hardest time with risk factors and how to assess them, diagnosis them, and implement plans. Are there any websites that summariezes everything in nice clean table or something?

get a good nursing care plan book. I use the one by Meg Gulanick and Judith Myers and I would recommend it. you can go to amazon and search for nursing care plan and that is the first book that will show. It gives detailed generic care plan based on the medical diagnosis, but make sure you use that as a guide and modify that to fit your real patient's.

See the link below, this is my favorite book for care planning because it lists medical diagnoses/conditions, what to assess, possible nursing diagnoses for them, and nursing interventions. It's small and compact, about the size of a drug book, and way more helpful than any of the nursing care plan books I have used.
