You still have to meet BON requirements for foreign trained nurse before they will allow you to sit the exam. Their requirement for foreign trained nurse is CES or if have taken the CGFNS exam in the past then that can be used.
Remember you are applying for license to be a RN in the US NCLEX is just a part of that process. You still have to meet their needs and from reading their site the only exception to CGFNS exam or CES is that you hold a US RN license and have maintained a clean active nursing license for 5 years. There is no such thing as an application for the NCLEX exam, that is why you are unable to find one. You will be applying for licensure as an RN to the state that you want.
MI no longer requires the CGFNS exam, but you need to have a CES, which is a credentials eval done before they will look at your application.

If you plan on working in Michigan then you need to apply to the state BON and meet requirements for foreign trained nurse. Links to the BON can be found at the bottom of the page

Hi I'm from the Philippines and i want to apply as a nurse in Michigan,i have heard that they don't require CGFNS anymore.i have check the Michigan board of nursing website and i have difficulty of searching the application form for NCLEX. If you have any information about the website where i can download the application please help me,any info would be very much appreciated.thank you very much.
