A patient with a large DVT is admitted to your unit. Heparin drip is to be initiated at 12 units/kg/hr with a bolus of 6000 units IV. Your drip concentration of 25000 in 250. patient weighs 310 pounds. how fast should you start your rates????
First, good job! All I did was ask a few questions to help get/keep you on track. The key in solving this type problem is determing what, of all the information provided, do you actually need as as they frequently provide much more information than you actually need.
As to my answer, I get 16.9090. This might resulted nased on how and when you rounded. In my practice, I don't round until I have completed all of me calculations and have obtained my final answer.
As to rounding. Where I work, i would program the patients weight, dose, and medication concentration into the pump and it would calculate thd rate automaticlly. If I was going to enter the rate, I would rounf to either 1 or 2 decimal places depending on whether I was using an infusion pump or syringe pump. As to how you round, you need to clarify this with your instructor.
