Call whomever carries your car/house/renters insurance. They can help you find health insurance.

Your state exchange, if your state has one, or the federal site which can tell you that too, will have links to help you, including showing whether you are eligible for premium assistance.
As a self-employed person I have my own via an association of small-business owners. If you are not employed by someone else you may also be a member of a group that offers this assistance.
For all of these, call your local general insurance agent (in the local phone book or equivalent) and have him/her do the work for you. It's a great service that a lot of folks don't know about.
