what happens when you are terminated from a nursing position/

Does it show up on "file" somewhere? Do you have to tell prospective employers about it??

anyone know?

There are no files anywhere. Smaller towns sometimes have gossip mills where there are no secrets. There is no need to tell another employer you were fired. Obviously you can't use that employer as a reference. When giving your work history, you might have to explain during the enterview why you left that, but you can sugar coat it and rather than say "I was terminated for........." you can offer another softer explanation.

A lot depends on the specific circumstances. For example, if you only worked at that job for a couple of weeks before you were terminated, it might be very easy to minimize it's impact on your future career. If asked about it, you can easily say something like ... "I briefly worked for XYZ, but that job was not what I expected it to be. It was a terrible fit and I only worked there for 3 weeks." ... or something like that.

However, if you worked for that employer for 10 years, a future employer is going to want to know why you left. They will also want to contact your former employer to verify your employment and may find out that you were fired. In such a situation, you'll need to prepare an explanation for your temination that is honest ... yet ... that points you in "best light" possible. If you made serious mistakes, you may have to acknowledge them and discuss what you learned from the experience to reassure the prospective employer that such mistakes won't happen again.

You may want to discuss your particular situation with a career counselor ... or with some knowledgable person who can help you develop the best strategy for your particular situation.

You might also want to check out the book "Building and Managing a Career in Nursing" published by Sigma Theta Tau. That book has a section on re-building a career that has sufered some sort of set-back, such as being fired. A good nursing library would probably have it or you could order it online from Sigma Theta Tau or Amazon.com.
