Pandemic Awareness/Preparation

It has been my own personal project to follow H5N1 for the last 3 years simply because it interests me. Attracted to this type of information like a magnet, I've been watching this relatively new influenza virus to see where it will go, how it will change itself, and possibly change our world. I have followed its country by country outbreaks, and watched for the important viral mutations such drug resistance or changes that allowed it to more specifically target mammals.

Keeping in mind at all times that we will be cleared impacted as HCW, as well as being members of our communities, and having families of our own to care for, I wanted to start the new year by opening a single focused pandemic thread that would also look at what we are doing nationally to prepare for a future pandemic. Is this the virus to spark the next pandemic? No one can answer that question. We can look back at the past to the last few pandemics, and in particular to the most devastating one in 1918, and extrapolate useful information about them, but we can not predict the future. We can only make comparisons with our situation now, and learn what worked to lessen morbidity and mortality in those past events. And, we can look at those other viruses, and compare them with what we are seeing now. For example, H5N1 is a Type A virus. We know that all pandemics are caused by Type A viruses. It is also an avian virus. The deadly 1918 virus, H1N1 was also an avian virus.

For this thread, as in the previous threads, I will be making use of news sources, scientific studies, govt bulletins such as the MMR, as well as flu forums and blogs devoted to this subject for my sources. Because press information, particularly the foreign press, is not always available for later access when I am looking back to check recent historical information, the use of these blogs and forums are important because archived information quoting the media and all other sources is always fully and easily available there with no worries about information disappearing or no longer being available. They also fully document their sources or I would not be using them.

With this link from Avian Flu Diary, a well researched source that I highly recommend, we can read the words of outgoing HHS Secretary Leavitt on our state of preparedness. Leavitt has done an admirable job during his tenure, but admits that there is much left to do.

A scant 33 months ago, I sent my first message about a race that HHS had just begun. As I said then, it was a race against a fast-moving virulent virus with the potential to cause an influenza pandemic. Since then, we have mobilized experts and resources across the country and around the world. I now send you this final message, as I look back at the unprecedented progress we have made in energizing a national pandemic influenza preparedness movement in those 33 months.

Today, many people mistakenly think influenza pandemics are a thing of the past, but influenza has struck hard in the era of modern medicine – much harder than most people realize. And it will strike again. Pandemics are hard things to talk about. When one discusses them in advance, it sounds alarmist. After a pandemic starts, no matter how much preparation has been done, it will be inadequate.

I just got home from Costco. I stocked up on necessities and spent over $400. I tend to do that with Costco trips, though, every few months.

I was feeling guilty, but you have me beat. Like you, I do not do this everyday either. I figure that I might not have the time to shop later.

I already had a lot of stuff. Now I have more...

The jury is still out on what is going on. Viral sequencing is going to tell us more on that story. I'm waiting for Dr.Henry Niman of Recombinomics to comment on this, but he seems to be busy with contact tracing and figuring out where the virus is going next.

He's very involved with that map project. I am really curious about those Mexican cases also.

Why have they waited so long to raise it to level 5. They say it is level 5 if it is confirmed in two countries. That occured last week I think!...Not that it would stop anything anyway, but it just seems...I don't know.

I am sorry, silvergirl. It is hard to comprehend this, I know.

Many people have been asking that very question especially the epidemiolgists. They were getting very upset that it took so long for this to be announced. If you listen or watch the WHO's press briefing, they attempt to explain the delay. And, it is obvious that they are expecting to go on to Phase 6 soon. They tell us this near the end of the briefing. It was a very somber meeting, and understandably so. I am glad that we are able to listen and replay what was said from the WHO archives.

While some in the healthcare community are still in shock, many do not even know about this, and a few are in denial. It is very hard to think about how this is going to affect us as we hear the stories coming out of Mexico. We are waiting to be told what to do. We wait for guidance in the workplace, and we are worried about our families.

It is going to be easier for us to deal with we might be asked to do if we start preparing for our own families first, and I strongly urge everyone to get started doing this as soon as possibe. There is much that has to be done.

I usually only have a couple chicken breasts in my frig. and that is about it. I may get some canned stuff if things start to look bad. With the mild form now though I just have trouble feeling that kind of urgency and I normally eat only fresh and healthy stuff which doesn't store well.

I have a question. When they do a nasal swab does it tell them if it is type A or type B influenza?

Thanks

Maria

A follow-up about my posting re: homeopathy. The National Center for Homeopathy has a flu forum:

http://nationalcenterforhomeopathy.org/services/crises/fluforum.jsp

You might want to explore the articles, which includes an archive of articles written over the years about flu treatment, and which name specific remedies to use, based on the symptoms.

Also, Dr. Herscu's perspective on this pandemic is very interesting. (Dr. Herscu is one of homeopathy's "heavy hitters", for those of you who aren't familiar with the field.) He says don't panic, because elements of this pandemic don't fit with what he thinks the "big one" (which is coming) will look like. You may agree or disagree, but his perspective is interesting. Also, notice that he says he has been seeing flu patients at Death's door for the last month!

I also wanted to post a link to Marge Clark's aromatherapy blog, where she talks about essential oils that may be helpful in warding off the flu:

http://naturesgiftaromatherapy.blogspot.com/2009/04/swine-flu-outbreak.html

She recommends inhaling the oil via a tissue; you can also diffuse those oils in a nebulizing diffuser.

The more tools for dealing with the flu that we have in our toolbox, the better, in my opinion!

Quote from Indigo Girl's CDC quote

"Sandman, who believes people should prepare for a more dire situation than is currently being seen, said the messages ought to be aimed to get people to understand the importance of hoping for the best while preparing for the worst."

There's great truth there! Much better than later debriefings of "shoulda, woulda, coulda"

Take good care y'all.....

HHS Sends 400K Courses Of Tamiflu To Mexico

http://afludiary.blogspot.com/2009/04/hhs-sends-400k-courses-of-tamiflu-to.html

The new HHS Secretary, Kathleen Sebelius, announced two new actions this evening regarding our strategic national stockpile of Tamiflu.

First, just a few days after authorizing the release of 25% of our national antiviral stockpile to the States, the HHS has announced they will order 13 million replacement courses of Tamiflu & Relenza.

Second, the HHS is sending 400,000 10-pill courses of Tamiflu to Mexico to help that country deal with their novel H1N1 flu outbreak.

The Strategic National Stockpile (SNS) has roughly 6 million courses of antivirals earmarked for possible early containment attempts in other countries.

In this case, containment is obviously impossible since the virus is no longer circulating just in Mexico, but there apparently remains some hope that these resources might slow the spread of the virus.

Dr. Margaret Chan -- swine flu ain't her first pandemic rodeo, folks.

http://www.scottmcpherson.net/journal/2009/4/29/dr-margaret-chan-swine-flu-aint-her-first-pandemic-rodeo-fol.html

Scott McPherson, the Chief Information Officer [CIO] of the Florida House of Representatives is always entertaining and informative.

After reading his blog entry for today, you can see why Chan is the head honcho at the WHO. I am only pasting the first part of the story.

A history lesson is in order for the newbies who are coming to this Website. Dr. Chan is a dual Canadian and Chinese citizen. I am confident that distinction has literally saved her life at least once.

In June of 1997, Dr. Chan was the public health secretary of the City of Hong Kong when the city was handed over to the Communist Chinese by the British government. Barely six months later, Hong Kong citizens began dying from a sudden outbreak of a new and novel strain of influenza. At first, Chan was told -- ordered -- by the Communist handlers to ignore the situation and to downplay it however possible. At first, she did this. But it was soon apparent that this novel virus was moving with devastating efficiency from person to person.

This virus was then typed. It was an avian flu that jumped the species barrier and began infecting humans directly. It was moving quickly and she recognized that drastic action was necessary. So, in direct conflict with her new Communist bosses, she disclosed the existence of H5N1 -- H5N1 -- and ordered the culling -- the destruction -- of all commercial poultry in the city.

Amazingly, her handlers stood down, the Hong Kong government moved quickly, and an H5N1 avian flu pandemic was averted. That original human strain of H5N1 was moving person-to-person and she stopped it in its tracks with her actions.
