Hello,
I'm new to this site and I hope that I'm posting in the proper place?
My grandmother is hospice and I'm hoping that I'll be able to get some help here. There is so many family members coming in and out and everything is getting very confusing.
We are looking for a charting system to use so that showers can be logged, meds given, meal prep. and so forth. The hospice we are using uses a computer program so I guess i have to make my own paper charting system. Any ideas as to what I put on it? Or where I can see an "example" chart ?
Your help is greatly appreciated.

If you PM me your email address I will send you a sheet in Excell format...all you will have to do is write the things to be in one column and the person who does it initials on the date they did it.
I also have a med sheet in Excell, will send it also.

You can also get a black and white copy book from the dollar store. Use that as a journal for everyone to record what happened during their time with Grandmom. I find that helps many familes in homecare/hospice.
11/29/02:
10AM Grandmom took meds. ATe only 1/2 breakfast. Aide here to bathe and made bed. Red skin on back---I called Hospice nurse. Sue

Hospice RN called said to apply lotion to skin and encourge her to change position every 2 hours-- she will visit tomorrow. Refused to eat but drank milkshake. Paul
10PM
Grandmom ate large bowl of spagetti with meat sauce at 8PM. Rubbed back. Helped her to Bathroom. Took bedtime meds. Insists on having light off in bedroom--cracked door so can see bathroom. Tell the nurse GM is having cramps in legs, I massaged them with some help. Jane
Hope things work out. Ask for a family meeting with Hospice RN to discuss some of your concerns I've found that the best way to improve communication along with med/treatment chart.

It seems to be that....... not all are reading my communication log because apparently there having difficulties w/ paragraphs.!
No, I'm kidding...I'm getting crabby w/ all this and I obviously have to have a chart if I'm to be in this situation for the next 4 months. I'd really appreciate it if you would send one, if you have one around ???
