I need help with interventions for risk of infection

Can someone help me come up with 5 nursing interventions for the nursing diagnosis Risk for Infection, and the achieved nursing outcome that I have to have is increased knowledge about the prevention and control of infection. The situation is care of the patient with a surgical wound. I am supposed to use the Nursing Diagnosis Manual (Doenges, Moorhouse, & Moor) for this assignment. Would I basically look at promote wellness (teaching,discharge considerations) that are under NURSING PRIORITES NO. 3? I am stumped!

Girl, you must buy a Nursing care plans book. Gulanick/Myers book is a great resource.

Nursing school is a long way and you should use other resouces beside your required class books.

Here are some ideas:

1- Meticulous handwashing before and after dressing changes

2- Use aseptic techniques(e.g, use of sterile gloves, dressings ect..)

3- Teach patient the importance of frequent hand washing and cleanliness

4- Administer the prophylactic/post operative antibiotic prescribed by the physician

5- Keep patient clean(e.g, gown, hair, skin ect)

Thanks for suggestions for nursing interventions. Before I ever posted for help, I actually was originally going to use most of those, UNTIL I looked at the nursing outcome that my instructor is requiring us to use,which is "increased knowledge about prevention and control of infection for a patient with a surgical wound" To me, that is saying that I must focus on interventions that increase or give the patient knowledge on how to prevent/control infection, NOT what I would do to prevent infection. Does that make sense? I am thinking that I would use more teaching nursing interventions, right?

Yes, focus on teaching. Most things listed above could be adapted to teaching the patient what to do, rather than just you doing them....

promote a healthy diet.....assess comorbidities in the pt..example are they neutrapenic because the recently had chemo treatment, do they have uncontrolled diabetes. Even treating the anxious patient who is having trouble coping with hospitalization (they tend to pick and mess with incisions). Also monitor vital signs and related labs. I think care plans are great because once you become a nurse these are the things that will rattle around in your head when it comes to knowing how to care for each pt. It gets easier.
