can someone help me to write smart nursing goals for .......1)altered mobility from spinal stenosis.....2) impared skin integrity from poor circulation......3)risk for hypoglycemia r/t diabetic

1) maybe... go to physiotherapy every other day for 3 months

2) walk for 20 minutes a day for 3 months

3) not sure about this last one

The ones you have so far look good for mobility but wouldn't you put it that they sould go to physiotherapy as prescribed? How about addressing the skin integrity, need to keep skin clean dry ... and then how about patient education?
