FSW Spring 2018

It's a bit early, I know. I just wanted to start a thread for all of those looking down the road planning to apply to FSW this fall for the spring 2018 program start..

I'm planning on applying to the Charlotte campus for the Evening/Weekend (ANEW) schedule format.

What campus are you planning on applying to?

What classes are you trying to finish up before applying in Sept/Oct?

What program schedule format will you be choosing (Daytime or evening/weekend)?

Did you take the TEAS VI exam and how did you do?

Okay but doesn't a heslth care provider have to fill everything out?? And for the second page does the doctor just do the mental and emotional requirements or both the physical and mental/emotional requirments? Thanks!!

My doctor had the nurse fill the first page out, and she signed it. I filled out the second page at the doctors office. I would leave the second page blank until you get the physical done.

We have to do a physical?? What paper is for that??

I'm sorry this has got me so confused! Is the back page the physical part?? Or doxwe have to do a separate paper for the physical? And what do we have to do for the physical? I have never done one...

The 2 page document is the physical exam form. Just take it to your doctor and make sure they sign/initial where it is highlighted and put their stamp on it. My doctor signed and initialed where it asks for a healthcare provider signature.

I filled it out beforehand. For the checklist, I just wrote the letter "Y" for each one.

Omg really?! I went to the health department in naples and they told me I have to do a physical for 50 dollars!!! And I had to do my Hep A vaccine and I can't do the second one until June!

I'm quite sure we don't have to get the Hep-A vaccine. I didn't get it. There wasn't any requirement for it on our castle branch medical organizer checklist and on our health report sheet there isn't any directions listed under it, like the rest of the categories.

For what its worth, my doctor charged me $85 to do the physical and PPD, and that's with partial coverage of my insurance. So $50 isn't too bad..
