Florida license with criminal history

In need of some clarification. I recently applied to the Florida state board, I originally tested in Ohio last month (March) and passed my NCLEX RN ! When applying to Florida they consider wreckless operation as a criminal offense and I submitted all the required paper work and am waiting to hear back if they grant me a license to practice in Florida. When doing research about This I came across an "exemption form" but it says I need a license number first. So my questions are, where do I submitted the form ? Does it cost money? How long does that take to come back ? Can I apply for jobs while awaiting the results ? Do I have to resubmit my criminal offense paper work with the form? I tried to call the ACHA who regulates the exemption forms but I was hung up on multiple times

Its a misdemeanor in Florida just as its is in Ohio.

The exemption form is only for nurses who already possess a Florida license.

I doubt your license will be denied for a simple reckless driving charge.
