I don't think we are clear on what you are trying to ask the patient.

Patient has been d/ced,let"s say, today from hospital and you have to call him to set up your visit for tomorrow. What you're gonna tell him?

Hi Mr Smith? This is Libby with Best Home Health Agency, we have received a referral from Major Hospital to provide a visiting nurse and physical therapy following your illness/injury/surgery, I'm calling to schedule an initial visit for tomorrow.

And if necessary..

Patient: I didn't know about this, why do you need to come?

Me:Your Dr wants a nurse to listen to your lungs a couple of times a week for the next couple of weeks and make sure the fluids/infection are resolving and to catch and report any early symptoms that we can deal with at home and keep you out of the hospital.

Stubborn Patient: I feel fine, I don't think I need a nurse.

Me: Hopefully you'll continue to have a good recovery, the hospital dr is concerned of a relapse and with patients sent home so soon these days, a visiting nurse is an alternative to staying longer or going to SNF.

I almost always use the term visiting nurse, it's makes for the simplest and clear description. And I explain our services in terms that benefits the patient and/or caregiver in ways they can identify, particularly being sent home early and staying out of the hospital.

I also try to add that the initial visit involves some extra paperwork and will be roughly X length of time, but that subsequent visits will be more brief.
