Fingerprint card

Hi! Just want to ask where can i get the fingerprint card and how much is it?

And do i really need to request for interim permit? what is it for? I would be taking NCLEX-CA..

one more thing., should i fill the form infront of the nbi officer or i can fill it up before i'll go to nbi? thank u
