Failed clinicals, how can I word my application to get into another school?

Dear Nurse Beth,

Failed out last term, due to clinical. My instructor said that I needed more time in clinical. I have already repeated. I am going to relocate to another state, and would like to join their program. I would like to be honest, but need help with wording to ask if institution will have me

I'm sorry you're going through this, and I wish you the best of luck getting accepted to another school.

If by "join their program," you mean joining an existing cohort, it's more likely you will have to apply to their nursing program and start over at first semester. 

Here's an example of words you could use if you choose to disclose your nursing school experience as part of your application. It's unclear how you failed out and then repeated clinical, but you could change that part if this doesn't represent what happened.

Subject: Inquiry Regarding Admission and Previous Clinical Experience

Dear [Admissions Coordinator/Program Director],

I hope this message finds you well. My name is [Your Name], and I am writing to express my interest in joining the [Name of the Program] at [Name of the Institution].

I recently attended a nursing program in [Current State] but faced challenges during my clinical rotation that resulted in not meeting the requirements for successful completion of the term.

In discussing the situation with my instructor, I was advised to gain additional clinical experience to develop my skills further. I took the advice to heart and repeated the clinical component to ensure I was better equipped to succeed in nursing.

I am currently relocating to [New Stat,e] and am interested in continuing my education and training at your esteemed institution. The [Name of the Program] would be an excellent fit as it aligns well with my career goals and values. I am impressed by the program's dedication to excellence and would be honored to be a part of it.
