Good morning everyone,
For the first time this summer, our camp director (new from this summer) as asked for an end of summer report for the health center. It's my second summer and I am the only health care provider. No one is really sure what they would like included in this report. Would anyone mind sharing what they do at their camp as far as end of summer reports are concerned? Being the only one here and already being quite busy with my regular daily activities, I just don't know where to start.

In my end of the year report I include: an average of how many campers a day the infirmary saw, how many kids went to see an MD, how many ER trips, mode of transportation to medical care (ambulance, camp vehicle or helicopter) a highlight of all serious injuries/illnesses, and recommendations for the next year-equipment needed, ideas to make the infirmary flow better.

If you had any sort of inspection (ACA, Health Dept., etc.) you should include the results; include copies of all Health Center/Health Officer specific policies and procedures, especially incoming/outgoing day procedures; a copy of your daily schedule; suggestions for changes.

A list of consumable supplies (band aids, alchohol wipes, Pepto Bismol) that need to be bought for next season or, if not currently stocked, added to inventory.
A list of forms (with supplied copies of said forms) that you need a high volume of copies of for next season, i.e. Patient Care Reports, program area first aid report forms, Medication Administration Logs, etc.
