what does an eligibility letter look like?
is like an nclex application?
to those who got their eligibility, can u please describe

There is no sich thing as an NCLEX application, so not sure why anyone still uses that term. You get approval on your application for Licensure as an RN to be able to even sit for that exam. It is impossible to apply for it.
The eligibility letter comes from the BON and states that you meet the requirements for licensure except for taking of the NCLEX exam. Nothing more than that. It is not the same thing as the ATT, which is the actual Authority to Test, and is sent out by Pearson-Vue.

last month, i requested an application packet from nevada, it contained the actual RN application form, two fingerprint cards and one envelope containing nclex examination registration form....
the instruction for it though, only send the NCLEX envelope once you the receive the CGFNS-CES report.
I was wondering if this envelope is the so-called "eligibility letter"?

no, what they are saying is do not send your application until your CES report has been sent from CGFNS, my CES report went straight to BON. Eligibility letter is once your application for licence has been received and checked to make sure you meet BON requirements. Then it will be a letter/email saying you meet requirments

Essentially what they are telling you is that they will not even look at your application until they receive the CES that is required.

It states that they have received and evaluated your credentials. It also contains your ID number and it clearly states if you are eligible to take the NCLEX if not.
