We do use Kardex's and though it is outdated... Sometime's I don't think I could care for the patient otherwise. So much gets missed in report that actually does show up on the Kardex. When we sign off orders, we make sure the information is on the Kardex also, if it's not we either write it on ourselves or give it back to the secretary before we can sign off the order. I haven't had any issues with things being missed on our kardex's. Until computerized charting comes to our town, I think it is the best option, at least for us it is.
We are currently using kardexes, but are getting rid of them before Dec. Yay!! So many times, things get missed, so when things get put into the computer, they are automatically updated. If it isn't in the computer, orders can't be signed off, so this makes it nice. I think kardexes are so outdated, unless you are working somewhere without computerized charting
We have kardex's on the computer and the actual old fashion Kardex where I work. They are trying to phase out the paper Kardex though. I like the computer version much better cause it gives the last 6 VS, 48hr I&O summary, procedures, treatments, vas access, current and future labs, ect.

Just wondering how other hospitals do with updates and all. I work at a facility and can't stand the whole Kardex thing, its so outdated and things get easily missed. What do you guys use?
