I just have a question concerning the topic on disaster planning in relation to the NCLEX-RN exam. The question is, if there is an internal disaster, who do you see first? the most unstable?, and if there is an external disaster, who do you see first? the least unstable?. I've been trying to figure this out before my exam but i'm a bit confused?

This is how I understand it :
When victims arrived in ED and they inside of the hospital - than assess the one that is most unstable (i.e. hemorrhaging , pneumotorax, shock etc). If you would use color system for triage than : Red (most unstable), Yellow (with minor damages, but not critical i.e. minor fractures, or minor lacerations with relatively stable VS), Green (those who OK, stable VS, and can ambulate or move their limb)
If you are the nurse at the site of the disaster - than assess one who has least damage (i.e. minor lacerations, stable VS). But those who have lacerations over 95% of their body should be assessed and helped last b/c you might have not enough supplies, or necessary equipment to help them. Here the order is Green, Yellow, Red (normally these pts called blue, or black) (order is reversed).

I don't think it's completely reversed in an external disaster. You still treat those that need immediate help. But- those that are too far gone to help aren't treated. You wouldn't treat minor lacerations first.
