declaration of nursing education and or licensure

hi to allnurses,it's my first time to post a thread in here but i have been a member for quite sometime. It is such a good help reading forums in here..thank you!

Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..:(

Hi mate do I need to pass this 2 forms with me?or just nursing declaration don't have any experience yet except my RLE back in college.

What do you mean 2 forms?Declaration has only 2 pages and you have to submit both.

You meant record of nursing program?Record of nursing experience is applicable if you had at least 24 months or 2 years paid bedside experience in a tertiary hospital.

You have to send the record of nursing program and if they send you the declaration of nursing Education/license,fill it out send it back too.

Hello again. I talked to our dean and she passed me to the lady who was doing the nclex applications from our school. So she was asking as to how to breakdown (the format on breaking down) those subjects that are integrated, and how to come up with the 2880 hours. Can I get a feedback as to how your schools did yours? We from feu are having a hard time getting our requirements done due to the integrated subjects and total nursing hours. Thank you so much!

Hello mga kababayan! Im processing my application to the Lvn board and presently here in cali, for the live scan fingerprinting do i have to fill up a form? What did you do to acquire the live scan fingerprint? Thanks in advance for the help.

Yes you to have to fill up the form and bring it with you when you do your live scan and They required 1 valid government ID.good luck

Just download the live scan form from the BVNPT website (you applying here in Cali right?). Make sure you got three copies :) Then go to a UPS store then you're good to go! Don't forget to bring an ID ok?

Just put the hospital namedirector of nursing or the chief nurse,or the clinical instructor,address of the hospital,telephone and fax number, for the date started I just put the day I started my first clinicals of NCM 101 until the last day before semester ends.and for the hours worked,408 hours in all.BTW I did all my NCM 101 clinicals in 1 hospital so I didn't have problem with that,same as the rest.[/quote']

Hi dragon lady, im on the process of applying for nclex-LVN and i found this thread really helpful because i received the declaration of nursing form too. just want to clarify something regarding the date completed? is it until the last day of ncm 101 semester ends? Is that what you mean? How about ncm 102 and 103? You had your clinicals on those semesters too right? Thanks in advance. :)

Hello! Im just wondering after submitting the declaration of nursing education/experience do the BVNPT still asked for more documents from you or they will give you your eligibility letter? I already send my declaration form last Dec 18,2012 and according to the evaluator that i talked to on the phone theyre still working on those forms submitted during last week of october and first week of november. Im really anxious to know since ive been traumatized when the BON denied me to take the NCLEX RN..
