Hi! I have some inquiries for homecare manager out there. I work as a homecare field nurse myself.
There is an opening where I work now for a RN Case Manager. And I am interested in applying.
But I want to have a clear picture of how a case manager day like.
Do you go on the field everyday/week/month for monthly patient visit?
How many case loads you have?
What do you like being case manager?
What are the challenges you had encounter as a case manager?
Any inputs are greatly appreciated!

I typically see patients Mon-Fri. I make my own schedule.
At any given time, I have about 30-40 active patients on my caseload. I am the primary nurse for about half of them.
I love my patients. I like taking care of them in their environment and feel like I actually make a difference/might makes their lives a little easier.
Communicating with doctors/hospitals/insurance companies. Doctors who, for some unknown reason, refuse to sign the 485 or who don't call you back. Communicating with hospitals can be a challenge... arriving to someone's home for a scheduled visit only to find out they've been admitted to the hospital. Or, worse, your patient is discharged from the hospital and no one calls or faxes any paper work and then after hours you're getting called by someone asking why no one saw this patient today... For the most part, I love my job.
Yes, $0.40/mile. I claim the rest on my taxes.
I start around 8am, see patients in the morning and then head to the office for most of the afternoon.

Thanks Kel, I was wondering the same thing! One more question, though.....going from staff (PDN) to case manager, how do you negotiate pay? I have no idea what is appropriate and fair to ask/expect.
