I don't think there is a short answer to that one, but I can recommend a couple of excellent books...

Iwould judge by the rationales the nurse uses to justify his/her decision.

If you're looking for an "application level" assessment of critical thinking rather than theory or formal tools, I would suggest evaluation of the nurse's ability to:
1. Identify patient problems (can she integrate book knowledge with what she is seeing in the patient?)
2. Manage the problem appropriately (knowledge of physiology & underlying principles)
3. Prioritize nursing interventions/actions needed to manage the problem
These are actually listed in order -- with #1 being the most important.
