Confused with the answer....any suggestions???

A client is on isolation after receiving an internal radioactive implant to treat cancer. Two hours later, the nurse discovers the implant in the bed linens. What should the nurse do first?

a. Stay in the room and call for help.

b. Pick up the implant with long handled forceps and place it in a lead lined container.

c. Leave the room and notify the radiation therapy department immediately.

I am confused because in Saunders it says pick up the implant with long handled forceps and place it in lead lined container but the answer for this question is "Leave the room and notify the radiation therapy department". is it correct???

hi guys! i found this on saunders 4e page 662.

the client is hospitalized for insertion of an internal cervical implant. while giving care, the nurse finds the radiation implant in the bed. the initial action by the nurse is to:

1. call the physician

2. reinsert the implant into the lady parts immediately

3. pick up the implant with gloved hands and flush it down the toilet

4. pick up the implant with the long-handed forceps and place it in a led container

rationale: a lead container and long-handed forceps should be kept in the client's room at all times during internal radiation therapy. if the implant becomes dislodge, the nurse should pick up the implant with long-handed forceps and place it in the lead container.

*clearly in this question the answer is number 4. maybe when there is a choice to notify the radiation therapy department , that is more appropriate or correct now since they are more trained and for safety the nurse should leave the room immediately to minimize the exposure.

I HAD this question it's "B" there is risk to both of you. Forceps and lead lined container are in the room for just this reason.

B is correct. remember during the treatment this pt is place in a lead-lined private room and is on absolute bed rest. you will do B, then notify radiation department, again pt safety

b is the correct ans.because it provides safety not only to the patient but also to the nurse...

a. it is a do nothing move... if you will just stay there without doing nothing you will also harm yourself

c.never ever leave the patient without doing nothing

d. RNs are not allowed to put implants back in place..

calm the patient/let him keep still...put the implant into the lead container with long handled forcep....then contact physician or radiologist and document the occurence

as i ans. thousands of questions letter b is the correct ans same situation.. same question...you will have the last say yet my ans . would still be... letter b..Godbless :yeah:
