Working on my interventions for a pt with anemia r/t blood loss. (This is not my nursing dx)
Little background- pt has a new stoma(6mo) and blood loss is thought to be caused by a large incision found on the stoma. Blood loss was sever so the incision on may have continued inside of the pt
One of my interventions is to monitor the sight for signs of active bleeding. I am having a very hard time finding a rational for this intervention. It seems like such an obvious thing and the few rationals I have found a simple one liners and this instructor doesn't seem like the type to take a simple answer. If anyone happens to find anything I would truly be thankful.

Maybe specify what ARE the s/s for active bleeding. Shoot, that could probably be multiple interventions, monitoring labs, watching stool, and how often you take vital signs & physical assessments
Yes I already have every intervention and rational... except for the rational for why I should monitor bleeding, nothing gives a good rational except 1 liners that are very general and this instructor would not like that. That's what I'm looking for help with, the rational for why I would monitor for bleeding

I bought the nursing diagnosis text and handbook. They have everything you need to complete a concept map. The text even had a chapter on concept maps. Most nursing diagnoses come with goals, interventions, related to, etc. I didn't have it my 1st semester and I hated doing the concept map. Once I purchased those books, doing concept maps were less painful. It took me a few to figure out how to use them. They handbook can be kind of confusing. Some nursing diagnoses are very detailed, but others are short and sweet, but that's usually because they are similar to another nursing diagnosis that was detailed in another part of the book. Good luck in school.
I purchased these 2 books. Remember if you work while going to school, you can write ALL your school expenses off on your taxes.

I am having trouble visualizing this bleeding, but is there some way of quantifying the amount you are seeing? I am thinking of wound dressings that saturate in 3 cm, or some such thing. Or the appearance of the stool: frank blood, burgundy, currant jelly, melana, coffee grounds, etc.
And I know this is picky, but you would monitor the site, not the sight.
