Yes I already have every intervention and rational... except for the rational for why I should monitor bleeding, nothing gives a good rational except 1 liners that are very general and this instructor would not like that. That's what I'm looking for help with, the rational for why I would monitor for bleeding

Working on my interventions for a pt with anemia r/t blood loss. (This is not my nursing dx) Little background- pt has a new stoma(6mo) and blood loss is thought to be caused by a large incision found on the stoma. Blood loss was sever so the incision on may have continued inside of the pt One of my interventions is to monitor the sight for signs of active bleeding. I am having a very hard time finding a rational for this intervention. It seems like such an obvious thing and the few rationals I have found a simple one liners and this instructor doesn't seem like the type to take a simple answer. If anyone happens to find anything I would truly be thankful. -M
