hi nurses! i am going crazy right now..
i applied for the CES by CGFNS so i can sit for the NCLEX here in illinois. so as of now, my high school diploma and license validation from the philippines both say MEET ALL PROGRAM REQUIREMENTS. CGFNS is still waiting for additional information from the school where i finished by BSN. they asked for my maternal and infant nursing theory hours. i hope and pray that there will be no problems regarding my theory/clinical hours in all areas.
so my main concern as of this moment is about that part in the REQUEST FOR ACADEMIC RECORDS/TRANSCRIPTS form.

questions are:
1. i attended 2 schools in my college life. in the form, i did not include the first school i went to. i just put the name of the school where i finished my BSN. did i do right thing???
2. for the dates, i only wrote down june 2004 - march 2007 since those were the dates when i was enrolled in my school. (where i finished my bSN). that would only total to 3 years. i did not include the dates when i was enrolled in my first school... i finished the 4-year BSN course, i hope it won't cause any problem or delay with my CES...
so now, im thinking i should've called and asked CGFNS before i filled up the form... any idea about this concern??? i would really APPreciate it...

REQUEST OF TRANSCRIPT FORM FROM THE BON:
TO APPLICANT: Send this form to your basic school(s) of nursing. If you need to contact more than one school, this form may be reproduced. Transcripts are required from each school where nursing requirements or general education courses were completed. Transcripts must include all completed coursework, clinical practice of training and reflect the degree awarded. Your school may require a processing fee.
If you obtained any general education unit that fulfilled your BACHELOR'S DEGREE, you have to also submit a transcript from the other school. They have to make sure that you have the necessary units to become a RN-BSN, as to having an ASSOCIATE'S DEGREE.

Hello! I did the exact same thing and got my CP certificate with no problems. I'm assuming it's also the same for their CES service. :) I requested to have my previous school transcripts mailed to CGFNS as well, though.
thank you for the replies... so i have to edit my CES report by adding my old school in it right?? but what if my main school integrated the subjects/units i earned from my old school into the transcripts that they've provided?
Usually if the transcripts are intergrated they are accepted
