Care Plans Help Please! (with the R\T and AEB)

Hello! I'm struggling with one of my classes, when the teacher gives examples it makes sense but when we're left on our own, it's extremely difficult to know where to start.

Specifically, trying to understand the nursing DX r\t (what it's related to), aeb (then the signs and symptoms).

Does anyone have any pointers to make this easier?

You are so right, I purchased the book I have now when I was in my second semester and having a lot of trouble with care plans. But now I find myself using it very little, more as a reference when I get stuck. I will also use it to make sure I have a comprehensive nursing diagnosis list to turn in for my patient diagnosis. We have to write a care plan for our top three problems. And, yes instructors are a very good resource. I could not begin to count the times I have shown them my problem list and have them say "Have you thought about this..." and the light bulb goes off in my head.

I also use the Doenges book which is helpful. BUT, when doing careplans, don't forget to use Maslow's heirarchy of needs, or basically the ABC's of prioritizing. It will help you with your NCLEX if you start to go by it now. Good Luck!

Try to think of the related as something you as an RN can treat.

IE: Impaired skin integrity R/T immobility AEB 2cm reddened area on coccyx

You as a RN can treat the immobility. Hope that helps!!! Also Ackley book is great for care plans!! Good Luck in your upcoming semester.

Hello there...Lets just say that CARE PLANS ARE THE DEVIL BOBBY BUSHAY

I like Mosby's Nursing Care plans and I like Doenges, Moorhous and Geissler-Murr care plans...all of which can be purchased at Borders or books a million or something like that.

Oh and another really good one is Ackley Ladwig Nursing Dx Handbook a guide to planning care

Look at all the things wrong with them...if they have a hip fracture...go to back of ackley ladwig..look up hip fracture then go to the pages it says this gives you a NANDA nursing dx then look that up in the meaty part of the book and this will give you some of the R/t and AEB and you take that and pick what applies to your patient...until you get a little better at it.

Then sorta cross reference with the other books and you will soon get the hang of it.

You will learn to tailor them more for you patients and not go strictly by whats in the book.

The Nurses's pocket Guide: Diagnosis, Interventions, and rationales (9th ed) has been a great resource. You can look up the medical diagnosis in the back and it gives you a complete list of all nursing diagnosis that apply. Good luck! ?

Can anyone help with collaborative nur dx? I have to do six of these along with six regular nur dx. In fact, I'm not even really sure what a collab. nur dx is. And to make it worse, I have pre-clinical today and my instructor will probably not be there and they (meaning all twelve) have to be done by clinical tomorrow. Anyone with informative hints, websites, advice, etc? Thanks, I really, REALLY appreciate it.

The collaborative problem may be also known as potential complications within each disease. I.E.: atelectasis in a copd pt.

Our school had a "Careplan workshop" Tuesday.I have a sheet of notes she gave us if you will e-mail me your e-mail address I will copy them and send them to you. It should really help! Just remember: Problem as stated by NANDA (Diagnostic Category) R/T Etiology/cause (contributing factors) AEB Signs/Symptoms (Defining Characteristics) example: Impaired skin Integrity R/T Immobility AMB 2 inch diameter ulcer on L hip...... now when you have High risk you will not have a AMB. so it will only have 2 parts. but email me your email address and I will reply to you with the notes my instructor gave us I HOPE THIS HELPS!

The way I understand the whole care plan thing is as follows:

Nursing Diagnosis=Acute pain What is your problem? The patient is in pain

The r/t = why is your pt in pain (i.e. abdominal incision, migraines, whatever)

AEB= what is the pt doing to let you know they are in pain (i.e. guarding behaivors, vital sign changes, groaning, or states I am in pain)

We have to do NOC and NIC care plans. Was doing good until they made us go to that format, now on May 12, 2006 I think I will have a bonfire in my back yard and NIC and NOC will be on top of the pile.

We use the carpenito book for approved NANDA and in the back of the book you can look up the medical diagnoses and it gives you some suggestions for nursing diagnosis. For the interventions/rationales....I LOVE the mosby's care plan book. Rationales are right next to the interventions and lists them by category such as teaching, assessing.

I know this will probably sound nuts but I am in week 6 of my 1st semester in nursing school. What is NIC and NOC in the careplan books? I can not find the abbreviation anywhere. Also which Mosby do you feel is best? I bought the All in One so I am prepared for Psy and Maternity. Thanks for any info you can offer!!
