Care Plan for Maslow's love & belonging

My Pt 65 yr female- morbidly obese 400lb. CFH & renal Def. Only gets out of bed to have bowel movement, bingo twice a week, physical therapy 3 times a week. Has a cath. Very cognitive She has denial of obvious problems, difficulty in perception of reality, likes to make up stories that make her feel good about herself, grandioisity, hypersensitivity to criticism, projection of blame & responsibilty to the doctors. 3 sister that never visist, she states the one that has power of attorney is just waiting for her to die. She only calls to see if life insurance is paid. Delusion of cancerous tumor in abdomen (Defensive coping) (impaired Social interactions) and (social Isolation) .

Would this work? Imparied Social interaction r/t disturbed thoughts secondary to disturbed body image or defensive coping

Sorry, I know this doesn't help your care plan, but I'd be thinking that mental health referral might be in order.

Tenuous perception of reality, ideas of grandiosity, delusions? Amongst other symptoms mentioned...just sounds like she might have an underlying dx.
