Canada to USA - Visa/Work Application Questions

I'm a Canadian nurse (Toronto) looking to move to New York and work there permanently. 

The requirements I know of are:
- pass NCLEX 
- New York nursing license (NYSED)
- VisaScreen (from CGFNS)
- TN Visa (NAFTA work Visa)

Am I missing any other requirements? 

After I get all the requirements, can I apply to hospital positions if I am from Canada? I get the sense that most people apply to travel agencies for short term contracts since they plan to return to Canada or their original country. However, I plan to stay in the US so do hospitals hire international nurses with TN visas?

If I have passed NCLEX in Canada and have an active licence in Ontario, do I fill out the application by Endorsement or Examination?

I am trying to apply for RN licence in Arizona via Endorsement, but there is no option for Canadian provinces to choose from. Only USA states were in the option. 

Do I apply by examination and ask CNO to send the results to Arizona BON?

Yes, I went to the Arizona BON website and  rechecked their endorsement application just now and you're right. You should not apply for license by exam if you have passed NCLEX. You should call or email Arizona BON to inquire. There might be alternatives.

But these websites seem to be promising. I also googled and found these resources specific to foreign educated nurses including Canadians.

Also, I saw on AZ BON website, that if I am educated in Canada, I do not need CGFNS report. All I have to do is contact CNO and ask them to send NCLEX proof and licence verification.

It looks like the requirement for Arizona license by examination is not that complicated at all here https://www.azbn.gov/licenses-and-certifications/RN-lpn-examination-faq. Pretty straightforward. as well as the requirements for Canadians https://www.azbn.gov/licenses-and-certifications/apply-for-a-license

(It appears like the process is similar to endorsement since you're not retaking NCLEX.) So when you request the Verification of License from CNO, it also has the note that you passed NCLEX.

You could definitely sign an affidavit if you don't have SSN or Lawful presence documentation @ https://www.azbn.gov/sites/default/files/2019-06/affidavit-re-lawful-presence-documentation-ss-number-for-foreign-national.pdf

The CGFNS Visa Screen Letter (?Certificate) is also sufficient as per https://www.cgfns.org/licensure/arizona/ for Registered Nurse - Initial Licensure. The Visa Screen Certificate is needed at the Customs/ Border if you're aiming for TN visa. 

*I both applied for Maine BON & Visa Screen at the same time so I requested same information from my schools & CNO.
