Can any nurses please help me with GTPAL?

I have read few maternity books, Nclex study guides, and posts on this site regarding GTPAL for pregnancies. I'm reading different things and I am very confused. My nclex is coming up soon and I am just so scared.

Example:

Sara is pregnant at 30 weeks right now. She has 1 son born at term, 2 twin daughters born at 34 weeks, 1 elective abortion at 22 weeks.

Gravida= 4 (current pregnancy, term son, twin birth which counts as 1, and the abortion)

Para= 3 (term son, twin birth which counts as 1, abortion since it's beyond viability of 22 weeks even though it was elective)

Term=1

Preterm= 1 (twins birth count as 1)

Abortion= 1

Living= 3

Is this correct? For GTPAL, we count the number of pregnancies and not the number of fetuses?

I am confused about abortions as well because according to Saunders, it says abortions are included in gravida if before 20 weeks, included in parity if after 20 weeks.

So for example, should the gravida NOT INCLUDE the abortion since it's past viability??

Yes, number of PREGNANCIES (even some docs get that wrong), NOT the number of infants. The L will account for any multiples.

Also, when you're talking Gravida/para - para counts number of pregnancies that end with a birth, NOT number of infants. So in a situation of twins, it's still G1 P1, it will never be Gravida 1 Para 2 (para will NEVER be more than gravida).

I'd call this woman G4 P2. I was always under the impression that "para" referred to the number of pregnancies that ended in birth.

Regarding abortions, I'd say they're always included in the gravida since gravida is the number of times a woman has been pregnant. In order to have an abortion, you have to be pregnant.
