I accepted a job offer with a union hospital. I signed all the paper work, background check, on boarding, and everything else pre employment. I have been waiting for my license in that state for weeks and was told it’s going to be a few weeks more. My start date has been pushed back once already and is likely going to be pushed back two more times due to my license. Do you think the hospital could retract their offer? I just signed a lease on an apartment so I’m extremely worried now.

I believe for the reason that you are unable to start on time, yes. I may be wrong. But you don't meet the qualifications without a license.
I am licensed in another state and am just waiting for my transfer license to be processed. They’re so backed up that they’re not able to get to my application for a few weeks. Do you think they’ll really retract it if I’m now with the union? I would think it would be harder to restart the interview process and all the on boarding/background testing/everything

They can, but I doubt they will.

Yes, of course they can. However, if you continue with good communication regarding any delays and verbalize your excitement in getting started, its likely they will hang in there.

I wonder if it's the same union state that took fooorrrever to issue my license as they were only open like 4 days a week for like 8 hrs. Website- 4-6wks max .... IRL ALMOST 10 WEEKS. no reason or background issues to hold it up. It all worked out ok. ?
