Because my textbook is out of date and my school's library has the 2005 ACLS books.
- what voltage is selected for defibrillation? My text says to start at 200joules for 1st, then 300 for 2nd, and 360 for third - but doesn't talk about biphasic defibrillators.

marykay,
for a biphasic defibrillator, you do not stack shocks. You keep the joules at 200.
(we use biphasic defibrillators on all our military ac.)
