1. the order is for heparin 8000 units subcut now. the pharmacy sends the following dose of heparin.

heparin sodium

10,000 units/ml

multidose vial containing 4 ml

a. calculate the number of ml the patient will receive.

8000u (dose prescribed) x 1ml

----- =

10000u

8000x1ml(u cross each other out)= 8000

------ = 0.8ml per dose(#ml is asked in question)

10000

b. how many doses will the nurse be able to give from the vial?

4ml vial so 4/0.8= 5 doses per vial

c. if the order is changed to heparin 6500 units subcut daily, how many ml will the patient receive per dose? same as above substitute 6500 for the 8000u

d. how many doses of the above order would the nurse be able to give from the vial?

same as above

2. the order is for 0.015 gram of a medication im stat. the pharmacy sends a vial of the medication labeled 0.01 gram per 1.5 ml. how many ml will the nurse administer?

0.015gram x 1.5ml

------ =

0.01 gram

0.015x1.5ml (grams cross each other out)

------------ =

0.01gram

2.25ml

3. the order is for iv ranitidine 75 mg q 8h. the pharmacy sends the following vial of ranitidine. ranitidine for injection -- 25 mg/ml. how many ml will the nurse give?

see if you can figure out this last one, any questions just ask

You can do this StudentNurseMommyof3! Just keep it simple ok? Don't try to over think it.

Order divided by what you have on hand and the multiplied by mL in this case. All your questions that you posted are done that way.

Once you come to your correct answer you can just do simple division to find out how many doses are in a bottle.

:)

If you want to post the answers that YOU got and how you did them if your unsure we can take a look!

desired dose then multiply by vehicle (in this eg. the vehicle is 1 mL)

dose on hand

These are all the same kind of calculation:

1. The order is for heparin 8000 units subcut now. The pharmacy sends the following dose of heparin.

HEPARIN SODIUM

10,000 units/ml

Multidose vial containing 4 ml

a. Calculate the number of ml the patient will receive.

b. How many doses will the nurse be able to give from the vial?

c. If the order is changed to heparin 6500 units subcut daily, how many ml will the patient receive per dose?

d. How many doses of the above order would the nurse be able to give from the vial?

a. Calculate the number of mL the patient will receive. . .
8,000 units
(dose desired)
/10,000
(dose on hand)
x 1 mL
(amount the dose on hand comes in)
=
0.8 mL
(dose to give)

b. How many doses will the nurse be able to give from the vial?
4 mL
(amount in one vial)
/0.8 mL
(amount of one dose)
=
5 doses
.

c. If the order is changed to heparin 6500 units subcut daily, how many ml will the patient receive per dose?
6,500 units
(dose desired
)/10,000
(dose on hand)
x 1 mL
(amount the dose on hand comes in) =
0.65 mL
(dose to give)

d. How many doses of the above order would the nurse be able to give from the vial?
4 mL (amount in one vial)/0.65 mL (amount of one dose) = 6.153846 doses, rounded to

6 doses
.

2. The order is for 0.015 gram of a medication IM stat. The pharmacy sends a vial of the medication labeled 0.01 gram per 1.5 ml. How many ml will the nurse administer?

0.015 gram
(dose desired)
/0.01 gram
(dose on hand)
x 1.5 mL
(amount the dose on hand comes in)
=
2.25 mL
(dose to give)

3. The order is for IV ranitidine 75 mg q 8h. The pharmacy sends the following vial of ranitidine. RANITIDINE FOR INJECTION -- 25 mg/ml. How many ml will the nurse give?

75 mg
(dose desired)
/25 mg
(dose on hand)
x 1 mL
amount the dose on hand comes in)
=
3 mL
(dose to give)
