Best nurse brain sheet?

Hey fellow nurses!

So I am a total newbie nurse in ICU. Still on orientation and I’m on a mission to find a brain sheet to help organize my day. Which ones do you all use?

Specializes in ER, ICU, MS, SNF, OTC, Perianesthesia, LTC.

I have one that a lot of people on my unit ask to use for the new staff. When I get I will upload a picture for you. I created it for what I needed when I first started icu.

What I created is able to be in a tri-fold. The left side is for labs the middle is for your notes for your abnormals on your assessment and then there admission reason is at the top. On the right hand side is basically you're rounding so if they have a fully or if they have cancer which side you reposition to mine then you could Circle the time and then add your data to that Med X are below what your vent settings are on and then your doctors are up at the top with your allergies. In critical care we do assessments every 4 hours, or three times a shift and so that is why this was created the way that it is

This is an awesome sheet! The only things I would add are: Current continuous gtts, vascular access (location, type, size) and Pain/Interventions.

Yeah. I put access under thr assessment and meds/ infusions on the right. But I don't use it anymore. I did my first six months. Now it's folded napkin?

I'm a newbie ICU nurse too and made these two SBARs, one for usual report and the second for admitting a post-op patient. I hope its, helpful.
