Had a client today state that they didn't get their meds over the weekend.
After checking the MAR I saw that he/she signed that they got them, and put it out of mind.
At 1 pm today, client came and said that he/she didn't get their meds this morning, and is requesting them right then and there.
Checked MAR and client signed that he/she got them and took them, and another employee verified the signature.
Told client that the MAR stated that he received them, and that he signed for them. Client quiet for about 5 seconds and then blows up stating that they are going to call a lawyer, that they need their meds, etc.
This happens on a weekly basis where I work, but today was exceptionally brutal.
Anyone have this happen before? I work in drug rehab, and a majority of our clients are paranoid schizophrenics, so I am sure this is part of the disease, but dang, was it brutal. This client accused everyone under the sun of taking his meds, forging his signature, etc. He even went so far as to accuse me, but I nipped that in the butt immediately.
Just needed to vent a little. Had a rough day today!

Yikes, working psychiatric drug rehab. I have no words of wisdom to offer but your job is incredibly tough. I would think co-workers or a charge nurse or the psychiatrist might know a good intervention?

If this is new behavior for this pt. , sounds like the meds prescribed aren't "working" as well anymore.....so I think I would let the prescribing MD know about the change in behavior. I remember a schizophrenic pt. I worked with who had a fairly dramatic behavior change, and she was found to have a UTI . At the time, the staff was so focused on her behavior change and psych meds, we overlooked this as a possibility initally. She returned to her baseline behavior once she finished her Bactrim DS !

Oh..man....that would be terrible to have to deal with all the time. I would assume just from the info you provided that the meds were given since they were signed....nursing documentation 101...but....if I were that nurse giving his meds...I would be writing a short little nurses note in the chart or whereever I could to CMA. know what i mean?
