Hi, Does anyone have ideas or forms you already use to do the collection part (pre MDS) that you use for collecting the information you need to do your MDS? I am just starting and any forms anyone uses would be of great help.. If I can download them or e-mail them to me. THANKS ahead of time!:wink2:
at our facilities we just print out the MDS and use it as a working copy. each dept. can print their own page and use it. at another facility we used a template. we are required to keep our working copies for a year from the last state inspection. the new assessments of course are blank, but we print the last assessment for the following assessments and mark through the old answers in red. that way we have something to go by and can see without having to enter data in the computer if there is a change.
you can also try med-pass's forms (not an endorsement)

mds 3.0 weekly/monthly summary
... or simply grab the idea and customize your own.

Wondering what form others are using for the interview parts of the MDS that get done by staff when the resident does not complete the actual interview --
I know that 'most' residents are supposed to be able to complete it, but we never actually planned for this, and will need forms just incase it can't be done by the resident. Would appreciate any sample forms that anyone has to spare for an unprepared peer!! Will share my email with anyone whom has forms, so they can be emailed to me. Thank you so very very much.

The downloadable MDS 3.0 booklet is already formatted to do just that, a worksheet. Print only the page/s of the interview sections you need. Each page has the name, ID, and date on the header.
