Hi, Does anyone have ideas or forms you already use to do the collection part (pre MDS) that you use for collecting the information you need to do your MDS? I am just starting and any forms anyone uses would be of great help.. If I can download them or e-mail them to me. THANKS ahead of time!:wink2:

I basically took the MDS and "cut & pasted" the areas that I am responsible to complete, and made it into a worksheet for myself. I have them with me so I can refer to them while I'm gathering my info. and making copies, so I don't forget anything.
Example:
Flu/Pneumo. information: ____________________________
Abnormal labs & dates:______________________________
Medications, which are new orders, psychotropics & diuretics, if on anticoagulants, etc. _________________________________________________
Recent physician visits, hospitalizations, ER visits, and Falls
__________________________________________________
Skin & Wound issues:________________________________
And so on, and so forth.

Hi I have a form that is in MDS sequence that I just highlight and then write in meds. If you'll tell me how, I will send you my email address and then I will send it to you. I complete most of the mds for the rehab unit so I have adopted another form to accomodate the rehab/medicare meetings.
