ACA form use

For those of you who use the ACA Form 2 "Camper-Healthcare recommendations bt Licensed Medical Personnel", how do you use it? Does the nursefill out the form or does the PCP? Much of the info is a review of Form 1- do you keep them together or use Form 2 in a Kardex type system?

If you are walking around camp and you carry a bag of supplies, do you carry camper info with you- in a camp where you have another provider in the health room? ie- do you make and carry duplicates or cards of some type? Do you chart then or when you return to the health room?

The PCP fills out form 2.

I make a spreadsheet master list with the camper's name, age, DOB, diet, allergy, and hx. If you are computer savvy, you can import these fields to Excel from your camp software. If not, it used to take me about 3 hours to do 100 campers. I then email the master list to myself and store it in my smartphone. Or just print off a paper copy. In the field, I have enough info, with what is in my phone, to assess and treat minor stuff. For something major, I bring them to the health center and pull the full record. I write short narrative notes (don't forget your f/u note!) at the infirmary.

I am thinking of using Google Voice and Google docs to dictate nurse's notes out in the field this summer. I'll then email the note to myself, print it out in the health center, and sign it. We'll see if it works.

Less is more when it comes to what you carry. I used to carry the whole hospital in my backpack. No longer.
