There's a lot of free stuff out there. You just have to look. I found a lot of resources and since there wasn't a single one I loved I used them to compile my own little 4 page packet. The first page is the parent's consent, 2nd page is for the Physician to fill out, 3rd page is a checklist for the Physician, Parent, and Student to fill out for the student to self-administer/carry, and the 4th is just an Epi-pen reference page for my staff.

I'm trying to gather resources to send home about severe allergies, especially food and insect bites. So far most of what I've found that's free isn't something I can print and make copies of (16 pages, lots of color, etc). Found a few good things, but they all cost mucho moolah.
I just got a bunch of FREE fact sheets about lice to send home from NASN, does anyone know of anything similar for allergies? I can't find anything on CDC, FARE, or NASN websites. I haven't asked if I have a budget for this sort of thing but I'm guessing the answer is probably "LOL no."
