Hello everyone. Within the past few months I accepted a pediatric case from my agency within New York City. It wasn't until I started the case that the mother told me we would be traveling out of state for appointments .

I told the mother that the agency did not let me know that and I wasn't sure how it would work. I later called the agency to tell them I was not comfortable with going.

I figured that the problem was solved and they would have spoke to the mother. But then I get a call yesterday asking me if I will be going to the out of state visit on Wednesday. I told them no and now I have to go to work today and I know she is going to try to guilt me into going or possibly get angry.

How would you nurses handle this?

I was a bit afraid and I have to say that you all were so great at explaining the situation to me.

It looks like the insurance provides the transportation (which is great because I was a bit worried about how we were getting there) and I like the mom and patient so I thought I would just suck it up and go.

I think it will be a great experience. I looked at it like this: not many nurses get to travel out of state for free. Although it is for a visit the mom said we could probably walk around a bit before the van picks us up to go home.

The 2 hours back home is a factor for me because if we reach back to NYC later than I am used to leaving it will impact my ride back home because I take public transportation. The public transportation I take comes few and far between after a certain time.

How we were going to get there was an issue for me. But it has been clarified and the agency told me that the licensing will be handled.

Thank you. I kid you not you nurses explained the situation so clearly to me which allowed me to change my mind.

Sounds like the agency didn't do a good job of explaining it to you.... glad it's all been resolved!

If there were anything about the situation that bothered me or remained unexplained, I would simply tell the agency that I can not continue with the case. If they want someone to do something that is not within the ordinary, then they will explain that ahead of time. Not doing so only makes the agency look very unreliable to the clients. Surely the agency already realizes that you use public transportation and they take that into consideration when giving you assignments. If they do not, then you need to keep that in mind for the future. Ultimately, the individual must always look out for themselves.

Im glad I dont have to go out of state on a 4 hour trip. Making small talk for a 20 minute trip is hard enough.LOL.

Are the parents traveling with a Critical Care team in an ambulance?

If that is the case,they do not need you.

My vent/trach kiddos usually have the CC team(which includes the Rn and 2 EMT's,plus Mom) go with them to appointments.

Not always used plus this sounds like this a "basic" not a t/v "high tech" case. My trach-vent kiddos are not required to use a CCT ambulance unless the parent does not drive, for the same reason a CCT ambulance is not needed to transport to school just a w/c bus and a nurse.
