We have recently seen an increase in a maneuver where the doctor will use an SVE to push upward on the chin of an OA presentation baby to increase descent in the birth canal.

Unfortunately I can't remember the name of the manuever at this time.

I had this unpleasant experience when i had my first.

All i could do was yell WHAT IS HE DOING WITH MY BUTT! He never asked, never explained. :angryfire Untill i asked him much later at an office visit. I also believe it's what bought my my second degree up the privy parts tear. :uhoh3:

I Perhaps these residents are confused? Nothing a couple of fingers in their rectums wouldn't help clear up I am sure... the things people will do to women in labor.

I looked this up in my 20th Ed. Taber's Cyclopedic Medical Dictionary, and it said:

Ritggen's Maneuver

"A manual method of controlling the delivery of the fetal head. The non-dominant hand exerts pressure against the fetal chin through the perineum. At the same time, the dominant hand exerts pressure against the fetal occiput...."

We have a doc who started doing this THING when he got to us after his residency. He used it on any presentation. Not only would he put his gloved fingers in there, he would wrap them in one of those blue surgical towels FIRST. I asked him why he was doing that and he replied that they "did it all the time in residency, it speeds things up". Needless to say, I had a few choice words for him after that delivery, and went straight to the head of OB with my concerns. Patients were complaining about rectal pain after delivery and stuff, and it just wasn't necessary. Impatience is YOUR problem, not the patient's problem. And doing because everyone was doing it is not a valid reason for me. I told him to let me stick two towel wrapped fingers into his rectum and see how he likes it. It was a big to-do about it, and we ended up writing him up everytime he did it, and safety tracking tools when patients complained of rectal pain.

I have seen this done and it is very brutal almost to the point of rape wait until you witness a Dr forget the pt is going natural and does it. I am talking nightmares to this day.

Your residents better be careful! A texas nurse got 20 years in prison for inserting a rectal thermometer into a patients rectum. Two fingers, might get em 40 years!

Ritgen's maneuver is intended to be done with a gloved hand under a sterile towel and pressure is supposed to be applied through both the rectal and lady partsl wall i.e. completely exterior.

this manuever, done properly, does assist with delivery in a very controlled manner (i.e. avoiding tears from suddenly popping the head through the perineum), facilitates delivery and is not very bothersome to epidural'd patients.

tell your idiot residents to get their fingers out of rectums (except, perhaps, their own) and learn how to do this maneuver properly! Where are their teachers? Holy Cow!!!
