Need help and advice in applying for my license w/ criminal history!

I am in the process of filling out the application with the State to take the NCLEX and apply for my license. In 2001, when I was 19, I was ticketed for a Minor Consumption and later arrested for a DUI and another minor consumption b/c I was underage while driving under the influence. I got a lawyer, paid my dues and everything was taken care of. The application is asking that I have a sworn affidavit with all related details, dates and locations for my arrests. First of all, what is a sworn affidavit? Do I need a lawyer for this? Where do I get a sworn affidavit? Also, I am not sure of the exact dates as this happened so long ago. I have a copy of my background check that I used to enroll in the Nursing Program I took, but my lawyer at the time somehow combined my charges into 1 so that I would not have to pay as many fines and it would look better on my record so it doesn't have the exact dates of both the arrests. I guess I am just looking for any assistance or advice possible. Should I get a lawyer to get the exact dates, before submitting my application? Should I expect trouble getting my license in IN due to my past criminal history? This is so disheartening because I am a different person now and have not been in trouble since. Hello, I am wondering when you did your background check for school, did it come up on there? and when you say you paid the fines and all and taken care of, were you convited or put on supervision all of those make a difference of how you would proceed. Look forward to speaking with you.

Lovely1206, Thank you for your response. When I went downtown to get my background check for school, it came back with all three misdemeanors listed, however because my lawyer had them combined when I went to court, only 1 date appears. So I only know one of the dates. The answer to your second question is- I don't know. When this happened, I was put on probation, fined and given several hours of community service...all of which I completed. I feel like I completed my requirements and the case was complete. I don't know what the disposition is technically called. This is where I need help. I don't even begin to know how to go about getting all of the documentation I need.

Perhaps you should call the board and ask if someone knows what you need? Or check with a lawyer that specifically deals with nursing issues. Good luck to you..

Most documents regarding arrest dates, disposition, etc will come from the clerk of court in your state. When you do obtain these types of document always have them certified and stamped
