I got thru NSO - mine is $1300/year for both adult/kids.

NSO (http://www.nso.com) and Marsh (http://www.proliability.com) seem to be the two companies that get mentioned most often. I've had coverage with both over the years but, since (fortunately!) I've never had to use the coverage, I don't have any opinion about which might be better.
