All, restraints do need doctors orders. In my hospital, restraints order need to be completed EVERYDAY. However, they would be last resort. On the NCLEX, pick the action that is least invasive and cause the least harm. For example, if the pt is agitated, instead of using restraints, try decreasing stimulation (darken room).

In addition, if a patient is agitated sometimes a sitter can be used or soothing music.

If a patient is disoriented, a sitter can reorient them or you can give someone with " busy hands " a job such as folding towels. ( at our facility a person with " busy hands " is one who likes to pull out their IV or pick at their dressings, etc )
