Immunization Record

Hi, I noticed that almost all the nursing schools required the submission of an immunization record to complete the admission. Can anyone tell me where/how to obtain one? Are they looking for a photocopy or an official document? I grew up in the states, so I have this little yellow card that basically had record all the immunization I've had. The secretary at my doctor's office told me that all I need is to photocopy the card, but it didn't sound too convincing. I have no idea where to start looking. Pls help! I'm from BC if that helps... hehehe

It should do. When our kids leave high school, all they get in my province is a green slip of paper with their immunization dates on it.

If you are an adult and have kept your shots up to date, we just get a cardboard card about the size of a credit card.

Don't worry until they reject it (if they do).

We have "little yellow cards" in Ontario.

Just submit what you have bearing in mind that you will likely need certain boosters ie) MMR, DPT, hep and probably a recent 2-step TB as a condition of admission. But that's worst case scenario.

Edit: photocopy is fine.

I moved a lot and never had a secure Dr. So I would have no idea how to get my record either.I hope your card works!

All I did was go to the local health center and they sent a request from the city in which I grew up in...I also work in the AHS so the occupational health nurse did a few immunizations for work, I just made a call to them and they ended up having my history on file...but then again even though I moved alot I still stayed within the same province so maybe that had something to do with making things so easily accessable :)
