Fingerprint Card Guide

hello. i found a guide in the California Board of Nursing website, but it lacks some stuff so i'll just ask it out here... I am referring to the California Fingerprint Card.

I don't have a social security number yet in the US, and in the fingerprint card it is marked X, which means i have to write something, what will I write? "None" or "N/A" or "Not Applicable"?

Also in Aliases or AKA, it is marked X, but i don't have any other names, what will i write? None still?

Date of Birth, do i write it in Numbers or in Words? Numbers like 01/01/07, 01-01-07 or January 1, 2007?

other Dates, is it in words or numbers?

Lastly, the Race. I'm Filipino so, it's Asian. What will I write? A? or Asian? where can i find a complete list of Ethnic Origins?

You are correct in all that you are asking, if you do not have it, then you cannot give it.

But also suggest that you do some more reading before sending in that application. It will be one month to get your results from the NCLEX exam, when you finally get permission to sit for it. You do not have a SSN#, so you will not get a license issued to you, and therefore will need to wait for a letter. There will be no other place to get your results, only be snail mail. And it still will not be a license in the US.

got hold of spare fingerprint cards from someone i know and it has the space marked "O R I" as blank.

what does this mean, do i fill this up or not?

Im currently finishing my Maryland application, my problem is about the fingerprint cards....will the NBI send it to the board or I will send it? how much it will cost for fingerprinting? sending it?

You send the cards, no one else is going to do it for you.
Maryland also has additional requirements, not sure if you are aware of them.

i applied for new jersey board of nursing i have some questions regarding my fingerprint card

1. employer and address...dnt have one... leave it blank?
2. reason to be fingerprinted? nclex exam or licensure for board of nursing?
3. miscellaneous number and sss? leave blank or write none?
