Is it better to be paid per visit or per hour?

I have an offer to work for a HH company... pays milage plus a per hour rate (only paid for time with the patient). It seems like paying nurses this way would only make them less producitve as they would be taking their good old sweet time at the pt homes. How do you prefer to get paid and why? which way is better?

If you are paid a per hour rate only while with the patient, hopefully the rate is adequate if a visit is only 30 to 40 minutes. I prefer per visit, there is much less record keeping of every moment of your day. I did work for a company that paid a straight hourly rate, whether with patients or not, but if you had no patients to see, you were expected to work in the office. That was too crazy.

I am currently in the process of transferring from an inpatient unit to home health within the same hospital system. They pay nurses the same hourly rate as in the hospital and the nurses are paid for ALL time on the clock (with patients and en route). We also get paid 0.51 per mile. I am very excited for the change and am thankful for being paid per hour (though I'll admit I don't have anything to compare it to). In my eyes I will be making the same pay (thank goodness!) plus a nice little mileage check, AND $25 for toward my cell phone bill per month. I feel as though I won't be rushed with the patients so much and trying to make more money by getting in more visits. While it would be NICE to do that sometimes (especially with Christmas coming up) I want to be able to be with the patients if they need it. I also like that as I am getting started (and going slower than others...likely with a smaller load) that my pay will not suffer!

Again I haven't started yet besides a ride along and a class about the OASIS but those are just my thoughts on what I have seen/read on this forum and heard from some friends in HH!!

Most people indicate that they do better with a per hour pay rate.

i have tried both and have found that hourly is best because i can give quality time to my pts. especially iv or wound vac pts, i am able to provide more education and troubleshooting techniques. that way they are calling the on call a lot less or not at all. the pts appreciate the time and in the end feel more confident in performing the care.
