Example:
"Mr. S., 76 years old, was admitted 2 days ago with a diagnosis of Alzheimer's Disease. Patient is disoriented x3. Mr. S. requires complete assistance with ADL's and is incontinent of urine. Nursing staff interact frequently with Mr. S., for reality orientation. Patient is restricted to the unit as Mr. S. wanders and gets lost."
I'm still a newbie so......what does x3 mean? What are other related terms that I need to be familiar with? Did a website search; found nada.

X3== person, place, time. Oriented X3 means you know where you are, who you are and the date( or close to it) Disoriented means you don't know.

We were taught that it's person, place and time of day (not date).

I worked with a neurosurgeon who told us not to bother with time other than month/year as he often did not know what day it was:chuckle

Sometimes "X4" is used, the 4th item being events (why they are there).

We just usually say that the patient is alert and orientated to name only......etc. I just love the phrase, "pleasantly confused'.....that's not so bad.......sometimes I think I"m 'pleasantly confused'..........

LOL - I love that phrase too. I think it describes me on a lot of days!
