Six Rights of Med Admin....HELP!

  1. I'm getting ready for my first Pharm test, and it seems as though I saw an acronym somewhere for the "Six Rights of Safe and Accurate Medication Administration".

    Do any of you know a good acronym to remember:

    "The right patient must receive the right drug in the right amount by the right route at the right time, followed by the right documentation."

    here is one I just made up:

    6R --> P DART D
    Right Patient
    Right Drug
    Right Amount
    Right Route
    Right Time
    Right Documentation

    Sorry... No good acronyms.. BUt if you just think about it, it should be fairly easy to remember...

    You need The right PATIENT
    The right DRUG
    The right DOSE
    The right TIME
    The right ROUTE
    The right documentation

    Okay dumb, but I remember it like this

    Dopey - Drug
    People - Person
    Don't - Dosage
    Remember - Route
    Things - Time
    Do they? - Documentation

    I actually learned it as 5 rights to, but it was pretty easy to add the last part to the acronym and it probably is good to remember the documentation as part of it too.

    We were taught 7.... I think Right to refuse is the 7th, but I will have to look in my Nsg 1 notes..

    I was tought 7 rights also. The 7th is right to refuse.

    We learned it as MDCRT (med cart).

    M--right med
    D--right dose
    C--right client
    R--right route
    T--right time

    If you wanted to add an E in there (MEDCRT)--the E would be for educate the client with the fact that you are giving him/her his meds.

    And everyone should remember to document automatically--practically everything we do in the hosp has to be documented.

    What- med
    When- time
    Where-route
    How much- dose

    we didnt learn the sixth sorry... But I guess you could just add it on there..

    I was taught the 7 rights in school. Patients right to refuse.
