I hate when you page a doctor over something pretty important, they give you a "fake" order, then get mad when you page them back. Hi, "give him some lopressor" isn't a real order, nor one I can act on. And you might want to hear the second part of my sentence...the part about the HR of 40...before ordering lopressor. But whatever. Don't flip when I page you again in

Yep I've had a couple o docs do that....the best was just last night give a beta-blocker and hung up......uhhh news flash a) I don't get to pick which one, nor do I get to guess how much and b) the SBP was already 70....so no beta blocker there :bugeyes: :bugeyes:.

This morning we get a patient from the floor with Afib with RVR. The Genius Intern resident wants to order a lopressor drip. I tried to explain that there is no such thing, let's start Amiodarone since patient is allergic to Cardizem. He called the Pharmacy anyways who says "Theres no such thing as a lopressor drip"....

I want to get one more mini-rant in before I go to work:

I don't want to go. Don't make me. I need some holy water to throw on these people (co-workers) or give me a mild anti-anxiety med to make it through the night.

I've seen Flomax ordered for women with bladder/kidney issues. By urology. It seems to work.

She didn't have retention...she had a stuffy nose with allergies.

I've had patients on metoprolol drips.

Side note: I actually had an order to give flomax to a femal patient that had a hard time urinating after a foley was removed! It worked!

I've used it for women with urinary retention and it does work, but my favorite Flomax orders ever are for men and women with foleys in place! :yeah:

How about this one: Dr shows up and orders Flomax for Mrs. Muggilitcutty. I laugh. He gets angry and demands to know why I'm laughing. "Doctor, Flomax is for people with BPH. MRS. M doesn't have a P to get H'd -benign or otherwise." "What DID I mean?", he asks. "Well, Doctor, I was absent on the day they taught mind reading in nursing school, but perhaps you meant Flonase." No joke. Actually conversation.

Only goes to show that doctors are humans too- they forget. Which is why there is the checks and balance of us helping them.

Here's my 'favorite' Flomax in women story. Nights had a woman who wasn't urinating. So the (male) nurse had a female tech cath her. The hurry-up-get-me-out-of-here covering surgeon breezes by while this is going on and orders Flomax. Nights left before the Flomax got there (not surprising, since it takes about two hours to get non-stat meds up). When I (day shift) went to give it I checked her B/p myself right before giving; it was in the tank. Then I looked at what the cath results were---50 ml. after 12 hours no-void. The woman was actually going into septic shock, and nobody was taking the time to put it together. She wound up in the unit but luckily made it. Ugh; gives me the shivers still.

Actually, anticholinergic drugs like Flomax are commonly prescribed for people with indwelling urinary catheters because they prevent painful bladder spasms and leaking around the catheter, and help prevent bladder shrinkage and thickening of the bladder walls.
